Hazard underwent surgery after fracturing his ankle when on international duty with Belgium in June, consequently missing the start of Chelsea's Premier League campaign.
The winger has since returned to action and helped his national side qualify for next summer’s World Cup.
On receiving a nomination for the prestigious award, Hazard said: "I missed football for three months. It is not easy to come back.
"I worked during the summer. I just want to play football. I just want to play football and try to bring the best out of me."
Hazard has looked bright since returning to Premier League action, but also admitted it is too soon to say whether he is back to his best. He said: "I don't know. I will say at the end of the season."
