The club confirmed the news on Friday morning, with fellow Italian Maurizio Sarri widely expected to replace Conte imminently.
Conte had a successful two years at Chelsea, but a fractuous relationship with the 'Blues' heirachy made his position untenable.
In his first season in west London, Conte helped Chelsea cruise to the Premier League title and he has followed that up with an FA Cup win in his second season. For context, that is exactly what Pep Guardiola achieved at Manchester City over the same two year period.
In a statement, Chelsea said: "We wish Antonio every success in his future career."
Conte was appointed Chelsea manager after Jose Mourinho left, and has previously had successful spells in charge of Juventus and the Italian national side.
